Disclosure Application Forms for NACCC Child Contact Centres – now DBS no longer CRB
Disclosure forms with the CRB logo printed on the front & back pages will now no longer be accepted at the NACCC office and the Disclosure and Barring Service.
If we receive any of these forms now, unfortunately we will need to return them to be re-submitted on the new forms, therefore if you have any of the old CRB forms please destroy them. Please contact the NACCC office if you require any of the newly printed Disclosure and Barring Service Disclosure forms.
